**ALL PARTICIPANTS OF METROPOLITAN YMCA JAPANESE SCHEDULE PROGRAMMES MUST SIGN-UP AS A MEMBER OF METROPOLITAN YMCA SINGAPORE. HOWEVER, MEMBERSHIP IS NOT NECESSARY FOR METROPOLITAN YMCA JAPANESE NON-SCHEDULE PROGRAMMES.
**ALL EXISTING EXPIRED MEMBERSHIP CONSIDERED NON-MEMBERSHIP.
**PLEASE NOTE THAT FOR NON-MEMBERS, A MANDATORY FEE OF $10.70 TO JOIN AS "ASSOCIATE YOUTH MEMBER" WILL BE CHARGED UPON SUCCESSFUL REGISTRATION. THIS MEMBERSHIP IS RENEWABLE ON AN ANNUAL BASIS.

通常プログラム参加者・新規会員をご希望の方（更新含む）はチェックをしてください。（通常プログラム参加者はメトロポリタンYMCAメンバーシップへの入会が必須です。）お申込みと同時に会員登録と年間費$10.70がチャージされます。有効期限が過ぎている方は非会員に分類されます。会員は年間での継続が可能です。

, being the applicant's parent/guardian, hereby agree to let my child/ward participate in the Metropolitan YMCA Programme with the understanding that while every precaution will be taken to ensure safety, I will not hold the Association responsible for any loss, injury or damage that may be sustained during the Programme.

As a member, I agree to comply with and be bound by the By-Laws, Rules and Regulations of the Association. I agree to allow the association to use my personal particulars as and when necessary to facilitate my membership benefits and entitlements.

I permit my child's/ward's photographs taken during these activities to be used by the association in printed materials and other authorised media.

I agree to show my Membership card upon request and understand that membership at the Metropolitan YMCA Singapore is neither transferable nor refundable.

I agree that 1st and 2nd swimming/basketball lessons which have been cancelled due to bad weather will be replaced with dry lessons, and the 3rd lesson that is cancelled will be replaced with a swimming/basketball lesson, subsequently cancelled lessons will not be replaced.

プログラム中は安全の管理に最善の注意を払いますが、けが・紛失・破損が起きてもYMCAまたはプログラム実施者に対してその責をといません。

貴団体の運営が円滑に行われるよう個人情報を利用することに同意します。

またプログラム中に撮影いたしました参加者の写真、映像を印刷・インターネット等、各種メディアに使用されることに同意します。

要求に応じて会員証を提示すること、またメトロポリタンYMCAシンガポールのメンバーシップは第三者への譲渡ならびに返金できないことに同意します。

《通常プログラムのみ》水泳・バスケットボール教室において、悪天候の場合ドライレッスンになること、クラス中止又はドライレッスンが3回以上になった場合は、1日の補講日が設けられることに同意します。
